a. Please note down the five digit application number for future reference. Read the instructions carefully before filling the application form. Refer to the sample filled-in application form in pages 9 and 10. Note especially the right method to darken alphabet/numeral with HB pencil as shown in the application form.
b. First, write in black ink using capital letters the required information in rectangular boxes (wherever provided). Then darken the appropriate alphabet/numeral underneath each box with HB pencil. What you write in these boxes is only for your guidance and for verification that you are darkening the correct alphabet/numeral.
c. A machine will process the application form. The machine picks up only dark pencil marks. Therefore, darken the alphabet/numeral using HB pencil only. Please see the filled-in sample application form (page 9 and 10) to learn how to darken the corresponding alphabet/numeral. d. If you wish to change a marking, erase the darkened spot completely and then darken the appropriate alphabet/numeral.
e. Do not scribble, smudge, cut, tear or wrinkle the application form. Do not put any stray pencil marks anywhere on the application form. f. Do not write or mark on the Barcodes.
g. Your photograph, signature, and address are to be scanned by a machine that recognizes only good quality images, from the specified areas of the form. Therefore, paste a good quality colour photograph with light colour background taken not more than two months earlier, and put your address and signature in black ink only.
h. Please note that your name, your parent's / guardian's name, and your date of birth should exactly be the same as given in your High School or first Board/Pre-University examination certificate. Any deviation, whenever discovered, may lead to cancellation of your candidature.
i. Your application must be complete in all respects. Incomplete application or application filled in a language other than English will be summarily rejected.
j. Choice of entrance examination city filled in the application form cannot be changed at a later stage.
Name of the Candidate
Write your name in capital letters as given in your original certificate of the High School (10th std.) or your first Board/Pre-University Examinations. Write a single letter in a box. Do not leave any blank box within any word of your name. Leave one and only one blank box between consecutive words of your name. If your name has several initials, leave one blank after each of them. If it requires more than 34 boxes, darken the * in the 35th column of the first row. Darken the appropriate alphabet/numeral underneath each letter of the name.
Category
Write your category GE, SC or ST in the space provided there and darken the appropriate box. Category once declared cannot be changed at a later date. The codes for different categories are as follows

Persons with Disabilities (PD)
Persons with disabilities (viz., locomotor, visual, speech and hearing) including leprosy-cured persons, who are otherwise fit to pursue the course, shall darken the box marked ‘yes’. All others will darken the box marked 'no'.
Gender
Darken the appropriate box.
Nationality
Darken the appropriate box.
Date of Birth
Enter the date, month and year of your birth as per the English Calendar and as recorded in your School Board/Pre University Examination certificate. Use numerals 01 to 31 for DATE, abbreviations JAN, FEB etc. for MONTH, and the last two digits of the YEAR of birth. For example, if born on 6th March 1985, the date should be entered as follows: 06 MAR 85. Darken the appropriate boxes for date month, and year in each column.
